This certificate shows you are eligible for reduced payments when receiving high cost medical care, such as when hospitalised.
Use
Show this certificate with your Health Insurance Card at the counter of the medical institution you attend. When paying for treatment you will only need to pay up to the "self-pay" limit, which is based on your income, there is also a limit for the cost of meals while hospitalised.

Generally apply for issuance through your employer.
