Summary
This certificate shows you are eligible for reduced payments when receiving high cost medical care, such as when hospitalised.
Use
Show this certificate with your National Health Insurance Card at the counter of the medical institution you attend. When paying for treatment you will only need to pay up to the "self-pay" limit, which is based on your income, there is also a limit for the cost of meals while hospitalised.
A way of acquisition
At the municipality office where you registered for National Health Insurance, bring your National Health Insurance Card, your seal, photo ID.
If you apply by mail, you will need an application form, a copy of your residence card and a postage stamp reply envelope.
If issued at the municipality office, you will get it immediately. If you apply by mail it will be mailed to your address at a later date.

If you pay more than the self-pay limit without presenting this certificate you may be able to receive a refund as a "High-Cost Medical Expense Benefit" and "Meals during hospitalization expenses"
