The card may also show: Name of the householder・Relation ・Permanent address・Status of Residence ・Resident register code・My number
Use
You need to show this card: when you start a job, as an attachment on official applications, when purchasing a car
A way of acquisition
You can apply at the counter of your municipality office where your address is registered, bring your "Residence Card".
Or you can apply by mail, you will need an application form, a copy of your residence card, a "Postal Money Order” for the fee, and a postage stamp reply envelope. For both application methods you will recieve your resident's card, with your My Number, by mail at a later date.

Nominally, it’s called "copy of Resident's Card (Juminhyou no utushi)".This is the resident's card itself.
