Your company or employer has to enroll you in order to receive the Type.2 Health Insurance(Company) Company Pension(Pension Type.2) and Employment Insurance by their requirements
An insured person or a person who was previously insured can complete a confirmation of qualification by themselves, as they might be disadvantaged at a later date if the workplace didn't report correctly or reported something that is not true.
You are required to pay the insurance premium when you haven't yet been enrolled but you were certified to enroll.
Application
Application for health insurance and national pension
Confirm with the pension office that controls your area by visiting them or mailing a letter to the office. You will need the below:
- A completed confirmation of qualification form
- A copy of your residence card
Application for employment insurance
Confirm with the hello work office that is controlling your area by visiting them or mailing a letter.
- Inquiry form (A hello work will have this)
- A copy of your residence card
