Use
Use it when recieving medical treatment (at medical institutions designated by the Japanese government for providing medical care at Health Insurance rates).
A way of acquisition
When you start a job with an employer who is obligated to provide government managed health insurance you will be provided with a certificate by your employer, it usually arrives around 2 weeks after you start work.
Fee
The card itself is free of charge, but insurance premiums will apply while you are enrolled, both you and your employer will pay towards your insurance premiums.
